This is where you provide your personal information and contact details (email, phone number, home address, and social security number).
ID proofing is used to verify your identity. You’ll need to provide specific information before you can enroll in coverage and get help with costs.
Who lives with you in your household? How old are they? What is their relationship to you?
Based on the information you provided in Step 1, the system
may already have your income information. If you think the number is accurate,
you can accept it. If not, you can fill in income information based on your pay
stubs, statement of wages, a W-2 or your tax returns.
You will be asked for additional information which will help determine program eligibility. Examples include, but are not limited to, other health care coverage, absent parents for children under the age of 21, enrollment in disability or retirement benefits, and others. If you are eligible for a subsidy, you will need to complete your enrollment on the NY State of Health Marketplace.
In this step, you will get an estimate of what your subsidy would be, and find out if you are eligible for premium tax credits to help you buy a Qualified Health Plan (QHP).
The system will display options and premiums based on the
income information you provided in Step 3. The plans you see and their prices
are based on any tax credits or subsidies you may be eligible
