The online LOI form requests standard information about your organization, plus the following information that may require some preparation:

1. Project title;
2. Primary contact information and legal signatory contact information;
3. Legal name of grantee organization (please spell out acronyms);
4. Organization web site address;
5. A brief one to two sentence overview of your organization;
6. The estimated total project cost;
7. The total amount requested from Affinity;
8. A statement of the problem your project will address;
9. The overall goal of your project;
10. An overview of your project and a discussion of how it addresses the problem you described above;
11. A description of how your project is innovative and a discussion of the rationale behind your project idea;
12. A brief description of your project objectives and corresponding measurable outcomes;
13. A brief description of the process you will use to meet your objectives (planning, executing and analyzing);
14. If appropriate for this project, a list of the potential partners you could work with that may be internal or outside your organization;
15. A list of the staff who will participate in this project and any staff you will need to hire including their job title, project function, and estimated percent effort;
16. A brief description of the other resources that are/could be dedicated to this project including matching funds, in-kind staff time, organizational support, etc; and
17. A list of the last three grants that you have applied for and whether or not your application was funded (please include date of each submission). We are looking to fund projects that are less likely to access traditional funding streams (National Institute of Health, Robert Wood Johnson Foundation, etc.).
