How do I return to my in-progress applications? Click on the following link (or copy and paste it to your web browser) to log in to your account and return to your in-progress applications. https://www.grantrequest.com/SID_626. Or you can click on the link in the "Welcome Grantseeker" automated email message that was sent to you when you created your user account.
What can I do if I have problems logging in, submitting my online application, saving the application, etc.? Call Staci Heldring at 718.794.5593, she will help you resolve your problem.
Who can apply for a grant? Providers in the Affinity network and or local community-based organizations and public health researchers are ideal candidates for this grantmaking program. We would like to see multi-disciplinary and or multi-site collaborations that represent all of the stakeholders needed to solve a complex problem. Affinity Health Plan will consider projects that are focused on the areas where our Members live.
What does Affinity fund? Affinity will fund projects in the following priority areas: Asthma, Chronic Pain, Depression, Diabetes, Health Literacy, Improving the Process of Care Giving and Care Seeking, Hypertension and High Blood Cholesterol, Multiple Chronic Conditions, Obesity and Teenage Pregnancy.
What doesn't Affinity fund?
  • National membership organizations
  • Annual campaigns
  • Religious organizations for religious purposes
  • Political activities, lobbying, or support of candidates for political office
  • Biomedical / Pharmacologic Research
  • Individuals
  • Emergency financial assistance
  • Conferences or fundraising events
How can I apply? First:  Read the "Grantmaking Overview" section of our website to determine if your project is eligible and fits with our strategic vision.

Second:  Eligible organizations must submit a brief letter of intent (LOI) electronically through our web form. The form requests information about your organization and project. After you have completed the form, you will be ready to submit your LOI by clicking the on-screen “submit” button.
What are the major deadlines I must meet? LOIs are due to Affinity by March 30, 2006. If you are invited to submit a full proposal, full proposals will be due 6-8 weeks after an invitation is sent.
What are the criteria by which Affinity will evaluate the proposed projects? To make funding decisions, Affinity first determines if your organization/ project is eligible and then assesses your LOI against the following selection criteria: Problem Statement, Innovation, Collaboration / Partnership, Narrow Funding Opportunities, Experience, Organizational Commitment, and Project Design.
How long does it take to review an LOI or a full proposal? Approximately 6 to 8 weeks.
Who will review my LOI or full proposal? A committee of Affinity staff and a team of professionals with expertise in a priority funding area or public health research.
When will we hear from Affinity regarding our LOI or full proposal? You will receive an auto-generated acknowledgement email upon submission of your LOI. In 6-8 weeks you will receive a funding decision letter.
What is the average grant amount that you fund?

Full Proposal Grantees:  Up to $135,000 a year for two years ($270,000 total award).

Technical Assistance Grantees: Up to $35,000 worth of technical expertise for up to one year to enhance the project proposal.

What should I expect during a site visit? CHI staff will set up a one to two hour meeting at your project site, if possible.  We would like to meet your project team and other key staff. There is no need to prepare a special presentation, we want to see how your project team operates and have a conversation about your work.
May I submit attachments with my request? Do not submit any attachments with your LOI.

If I submit an LOI online, should I also send a paper copy?

No.
Does Affinity allow an indirect rate as a line item in the budget? Yes, we will consider up to eight percent (8%) for indirect costs.

If I have submitted a request and been declined, can I resubmit?

Yes, please resubmit during our next funding cycle.

When and how are funds released?

Affinity will release payments based on agreed upon milestones.
