Introduction:
Affinity Health Plan is looking for partnerships between
community-based organizations, providers, public health
researchers and others to test “outside-the-box” ideas
that have the potential to generate new knowledge and
improve the health and healthcare of our communities. We
would like to fund projects that represent all of the
stakeholders needed to solve a complex problem.
Strategic Vision:
Affinity is deeply concerned with the disparities that
exist among and across populations, access to care, and
the health status of the population. Because of this,
our Board established the following goals for
grantmaking, under the Making a World of Difference
program:
- Reduce the burden of
illness, injury and disability
- Improve the health
and functioning of people in our communities
- Deliver healthcare
that is safe, effective, patient-centered, timely,
efficient and equitable
- Focus attention on
programs that address/reduce disparities
- Focus attention on
programs that improve health literacy and,
therefore, improve clinical and other outcomes.
Priority Funding
Areas:
Affinity seeks to fund projects in the following
priority areas:
- Asthma
- Chronic Pain
- Depression
- Diabetes
- Health Literacy
- Improving the
Process of Care-giving and Care-seeking
- Hypertension and/or
Cholesterol Management
- Multiple Chronic
Conditions
- Obesity
- Teenage Pregnancy.

Geographic
Eligibility:
Affinity Health Plan will consider projects that are
focused on the areas where our Members live. Affinity
serves residents of the five boroughs of New York City,
as well as the five surrounding counties: Nassau,
Suffolk, Westchester, Rockland and Orange.
Funding Exclusions:
- National membership
organizations;
- Annual campaigns;
- Religious
organizations for religious purposes;
- Political
activities, lobbying, or support of candidates for
political office;
- Biomedical and
pharmaceutical research;
- Individuals;
- Emergency financial
assistance;
- Conferences or
fundraising events.
