To Affinity Health Plan’s Making a World of Difference program website.

The mission of Affinity Health Plan is to improve the health of underserved populations by providing comprehensive, affordable, high-quality healthcare coverage, and by striving, in collaboration with its primary care providers, to improve care-giving and care-seeking.

Affinity Health Plan has two mission-driven goals: to improve care-giving and care-seeking and to improve the health of underserved populations. Efforts to achieve these goals go beyond striving for excellence in our day-to-day business operations. Affinity’s Making a World of Difference program engages with others to advance these goals. We do this in several ways:
  • Grantmaking: To promote innovation in community health;
  • E-Alliance: To develop and promote new ways to use technology in our delivery system;
  • Learning & Development: To provide opportunities for our providers and partners to support life long learning.

GRANTMAKING:
Affinity’s Grantmaking program will provide funding and technical assistance to community based organizations, providers, public health researchers and others -- that are interested in testing innovative solutions to complex public health challenges. Competitive applicants will address one or more of the following objectives: preventing morbidity and mortality, reducing the burden of illness, reducing disparities, or improving patient functioning and outcomes. In addition, projects must focus on one of the following priority funding areas: Asthma, Chronic Pain, Depression, Diabetes, Health Literacy, Improving the Process of Care-giving and Care-seeking, Hypertension and/or Cholesterol Management, Multiple Chronic Conditions, Obesity, and Teenage Pregnancy. E-ALLIANCE:
Affinity’s E-Alliance initiative seeks to improve service delivery and health outcomes through technological solutions that align, coordinate and integrate the information needs and capabilities of Affinity, our Providers, and our Members. Our first step in this initiative is to improve the flow of communication and information with our Providers and Members through a collection of new services. To date, three new services have been launched: a portal for providers to confirm eligibility, check on claims status, and send secure messages; Electronic Claims Submission and an Interactive Voice Reponses System to confirm Member eligibility by phone in 60 seconds or less. LEARNING & DEVELOPMENT:
The Learning and Development Program offers a wide array of training and educational opportunities ranging from certification programs to customer service workshops and stress management seminars. Provider staff and the community at large will be able to access educational and training modules as well as on-line courses. Affinity views health as a very broad concept including physical, mental, and social realms of well-being. Recognizing this, Affinity has dedicated resources towards education and service delivery programs that will benefit our Members, Providers and the community at large.
