Affinity is deeply concerned
with the disparities that exist among and across
populations, access to care, and the health status of
the population. We are looking to fund projects in one
of these ten areas:
Asthma
is a disease causing inflammation of the lungs and
breathing difficulties. It is the most common chronic
disease in children, and the sixth most common overall
in the United States.

Chronic
Pain affects one in
three Americans, or roughly 50 million people. Pain is
affected by a complex combination of emotions, culture,
experience, spirit and sensation.

Depression
is a common and debilitating illness that affects all
segments of the population. Recent studies show that too
often depression goes unrecognized or untreated, and
many people still are not getting the help they need.

Diabetes
- Over the last several decades, the rate of adult-onset
diabetes has increased dramatically in this country,
prompting the CDC to label diabetes an epidemic. In
addition, approximately one-third of people with
diabetes do not know they have the disease.

Health
Literacy, a direct
priority of Healthy People 2010, encompasses a range of
skills including writing, listening, speaking, and
mathematical and conceptual abilities. Individuals who
are health literate “can obtain, process, and understand
[the] basic health information and services they need to
make appropriate health decisions.”

Hypertension and High Blood Cholesterol
are pervasive in this country. Each is a major risk
factor for cardiovascular disease (heart attack and
stroke), by far the leading cause of death and
disability in both men and women.

Improving
the Process of Care Giving and Care Seeking
- Today in the United States, there are major
disparities in people’s health status, health behavior
(such as diet and exercise habits), access to medical
care, and quality of care.

Multiple Chronic Conditions
- Nearly one-quarter of Americans have two or more
co-existing diseases, and face numerous health risks and
limitations. Coordination of care for people with
multiple conditions is lacking, with many patients
reporting difficulties in getting the care they need.

Obesity
- The rising level of obesity among Americans represents
one of the most worrisome trends in public health, with
alarming implications for health care costs, disease
rates, and life expectancy.

Teenage
Pregnancy - Across
the United States, more than one in three girls will
become pregnant before their 20th birthday, the highest
rate in the industrialized world. In New York City,
rates for teenage pregnancy and births far exceed
statewide and national averages.
