Who can join our Child Health Plus program? People who...

• are under 19 years old;
• live in New York City, or in Nassau, Orange, Rockland, Suffolk or Westchester county;
• are not eligible for Medicaid or Medicare; and
• do not have health insurance.

How much does Child Health Plus Membership cost?

This program is free for families with incomes less than the "full subsidy" levels on the chart displayed below...the government pays Affinity Health Plan for "full subsidy" Members. Depending on family income, Members may pay a "partial premium" or a "full premium." There are no deductibles or co-payments for covered services
