Who is covered by our Family Planning Benefit program? People who...

• are Child Health Plus enrollees of Fidelis Care NY, and live in Nassau, Suffolk, Westchester or Rockland county; or
• are Family Health Plus enrollees of Fidelis Care NY, and live in Nassau, Suffolk, Westchester or Rockland county.

How much does Family Planning Benefit program Membership cost?

This program is free...the State...not the Member...pays Affinity Health Plan. Members do not have to pay a monthly premium; also, there are no deductibles or co-payments for covered services.

What services does our Family Planning Benefit program cover?

This program is limited to the following family planning and reproductive services:

• advice about birth control;
• pregnancy prevention and testing;
• testing and treatment for sexually transmitted diseases;
• abortion;
• sterilization;
• birth control or other family planning prescriptions filled at a pharmacy; and
• HIV counseling and testing when done during a family planning visit.
