Claim Search Screen

To view claims status, follow these steps:

1.       Hover over the Claims tab.

2.       Click on the Claim Search pull-down option.

3.       Enter in the appropriate search criteria on the Claim Status Inquiry (Affinity Member ID is a required field.)

4.       The Claim Summary screen displays the search results. Click on the claim number link to view a specific claim.

5.      Review the Claim Detail screen.

6.       This page displays all of the high-level information about a claim including overall claim status, provider information, and billing information.

7.       To print this detail page, click on the printer friendly format link.
