Vision Services

The Block Vision Network provides routine eye examinations and glasses for our Members, as follows.

• Members under 19 years old may have a routine eye examination and new glasses every year.
  
  
• Members who are 19 and older may have a routine eye examination and new glasses every other year.
  
  
• Members may self-refer for optometric services. No referral or authorization from the Primary Care Provider is needed.
  
  
• Participating Block Vision Optometrists are listed in the Affinity Health Plan Provider Directory.
  
  
• Block Vision can be reached at 1-800-428-8789.
