    The Way to Renew Your Medicaid Coverage Has Changed

    Do you have Medicaid coverage? You have to renew your plan every year in order to keep your health benefits. You will receive a letter from either NY State of Health (NYSOH), the Human Resources Administration (HRA), or your Local Department of Social Services (LDSS), depending on how you first enrolled for coverage, letting you know when it’s time to renew. The letter will have your renewal date and instructions on what to do next. Your renewal date is the anniversary of when you first enrolled with Affinity.

    You will have 30-60 days from the date in the letter to renew your coverage. You must renew by the date in the letter or you will lose your benefits. If you enrolled through the NYSOH Marketplace, you will need to renew your coverage via the Marketplace. Log into your Marketplace account at www.nystateofhealth.ny.gov, or call the NYSOH Marketplace at 855.355.5777.

    If you have not received a notice, make sure your information is updated with Affinity and NYSOH so that you will not miss your renewal deadline. To update your information with NYSOH, call 855.355.5777 or visit www.nystateofhealth.ny.gov.

    Be sure to bring all required renewal forms and other documents when meeting a representative at a Community Service Center.

    Essential Plan Members
    • If you received a renewal letter from the New York State of Health (NYSOH), call Affinity’s Customer Service at 866.243.3174 for help renewing your coverage.
    • To make sure you’re covered on January 1, 2018, you have to renew between November 16, 2017 and December 15, 2017.
