• Middletown Community Service Center Closing

  • Affinity Health Plan’s Middletown Community Service Center closed on June 1, 2018. Members will receive the same great service at the nearby Newburgh Community Service Center at 149 Broadway, Monday through Friday, 9 a.m. to 5:30 p.m. Members may speak with an Affinity representative at the Regional Economic Action Program (RECAP) at 123 East Main Street in Middletown on Wednesdays from 9 a.m.to 5 p.m. and the Galleria Mall at Crystal Run, 1 Galleria Drive in Middletown, from Monday to Friday, 10 a.m. to 6 p.m.

    Members can get immediate help by accessing the Member Portal, where you can print ID cards, pay monthly premiums and change doctors. Members can also call Customer Service at 866.247.5678, Monday through Friday, 8:30 a.m. to 6 p.m.
