    A complete list of the treatments and procedures that require providers to obtain pre-authorization is available by clicking on the links below. This new list went into effect April 20, 2020 and will remain in effect until September 11, 2020.

    Please note that authorization requirements for Beacon providers have not changed and still go to Beacon. eviCore requirements go to eviCore; Superior Vision go to Superior Vision; Dentaquest go to Dentaquest. All scheduled inpatient admissions require prior authorization, as do all requests to use a non-participating provider and all requests for durable medical equipment that are billed for more than $500.
